Introduction

1. Sepsis screen
â–º Absolute neutrophil count (ANC; < 1800/mm3)
â–º Immature to total neutrophil (I/T) ratio (> 20 %)
â–º Micro - ESR (> 15 mm 1st hour)
â–º CRP +ve (> 10 mg/L)
*If two or more criteria are present, we label sepsis screen as positive
2. Absolute neutrophil count (ANC)
â–º Do TLC and DLC
3. Neutrophils

Count all neutrophils- mature and immature
4. Absolute neutrophil count

5. Immature to total neutrophil ratio (IT ratio)
â–º Mature neutrophils - 40, band - 5, prometamyelocyte - 3,
metapromyelocyte - 2, lymphocyte - 48, eosinophil - 2
â–º IT ratio: (10/ 50) X 100 = 20%
6. Sepsis screen
â–º Serum CRP: quantitative or semi-quantitative: >10 mg/ dL
â–º A separate video on how to do CRP
7. Micro ESR
â–º Take 75 mm microcapillary
â–º Heparinize, close one end and suspend vertically on the wall
â–º Measure fall of RBC column in first hour

8. Key message
â–º Sepsis screen is a panel of 4 tests namely ANC, IT ratio,
micro ESR and serum CRP levels
â–º If two or more criteria are positive, screen is said to be
positive
